Body Mass Index (BMI) is a foundational anthropometric tool used to categorize individuals into weight classifications (underweight, normal weight, overweight, and obesity classes I-III). It is widely used in epidemiological studies and clinical screening to identify potential risks for metabolic syndrome, cardiovascular disease, and specific surgical complications.

Clinical Pearl

BMI is a population-level metric, not a direct measure of adiposity. It cannot distinguish between fat mass and muscle mass. Therefore, a highly muscular athlete and a sedentary individual with high central adiposity may have the exact same BMI, despite vastly different metabolic risk profiles.

Pitfalls & Warnings

  • Inaccurate in pregnant women, professional bodybuilders, and patients with severe muscle wasting.
  • Falsely elevated in patients with massive fluid retention (e.g., severe ascites, anasarca, decompensated heart failure).
